The Club Sports Program provides a wide range of instructional and competitive athletic opportunities with an emphasis on student athlete leadership, organization, team management and sportsmanship.  For athletes who want to specialize in their sport and really hone their skills in that one sport, joining a club team could be a great way to get the coaching and sport-specific development they desire.

PRACTICE:  Sunrise Mountain High School, 4:30 – 5:30 p.m., January 2, 3, 7, 9, 10, 14, 16, 17, 24, 28, 30, 31; endurance group Saturdays 8 a.m.  SMHS 21200 N 83rd Ave, Peoria, 85382.  Additional practice days are communicated in advance by the coaching staff.  Schedule updates, announcements and general communications are done via the private mobile messaging platform REMIND. COMPETITION:  Most competitions are Saturday mornings (7 a.m. or 8 a.m.) with an occasional Friday evening or Sunday morning for events that are contested over two or more days.  ENTRY FEES range from $6 – $20 per meet and for some competitions per event.  There are usually four competitions (called “meets”) on the indoor schedule and eight including the state championships on the outdoor schedule.  Meets are contested at high schools and junior colleges across the greater Phoenix area and surrounding cities. Entry fees, travel expenses and transportation are the responsibility of the athlete.  The Youth, Open and Masters competition schedule is online at the association website –

ASSOCIATION MEMBERSHIP:  Athletes must be USA Track and Field (USATF) members to participate in USATF competitions (track & field and cross country).  The Youth membership fee is $20/year, Open and Masters is $30/year.  The AAG Sports Academy is a USATF Member Club since 2008, our club number is 48-0360.  Include the club number in your association membership application.  Complete your USATF membership application online –

UNIFORMS:  Team race singlet is $40 due the first week of the season.  Returning members can compete in their previous year’s uniform provided it is in good condition.

EQUIPMENT:  All practice and competition equipment is provided. Footwear is the responsibility of the athlete.

I am the parent or guardian of the minor child or ward registrant ("my child"). I hereby give my child permission to try out for and to participate in any and all programs associated with AAG Sports Academy ("AAG") including but not limited to AAG's baseball, softball, track & field, basketball, football, educational and mentoring programs and field trips related thereto ("Activities"). I understand that (i) despite the efforts of AAG to instill a "safety first philosophy" within the organization, my child may be exposed to certain risks and hazards incidental to his/her participation in the Activities, including, but not limited to, physical injury and (ii) the permission that I give herein means that I am agreeing to assume on his/her behalf all risks and hazards incidental to his/her participation in such Activities, including, but not limited to, physical injury. With respect to such risks of physical injury, I further understand that the permission I hereby grant means that I am agreeing to assume on his/her behalf all risk and hazards incidental to the Activities and all warm-ups, practices and competitions. In consideration of AAG permitting my child to participate in the Activities, I hereby release, discharge and agree to indemnify and hold harmless the AAG Youth Development Academy of Arizona, Inc. and AAG Sports Academy and their respective parent, subsidiary, affiliated or related companies and the officers, directors, employees, agents, contractors, sub-contractors, representatives, successors, assigns, and volunteers of each of the foregoing entities (the “Released Parties”) from any and all claims, liabilities or causes of action arising out of my child's participation in the Activities, including but not limited to physical injury to my child, whether the result of negligence or any other cause. PHYSICAL CONDITION I hereby certify that my child is physically fit for participation in the Activities specified above or other activities conducted in conjunction therewith and has the skill level required in conjunction with the Activity, and I have not been advised otherwise. EMERGENCY MEDICAL TREATMENT CONSENT I hereby give my permission to AAG to give consent on my behalf in the event of the need for the emergency administration of medical treatment which AAG, in its sole discretion, believes to be necessary and appropriate, including, without limitation treatment by trained First Aid personnel; EMTs; First Responders, Paramedics and Emergency Room Physicians. In consideration of AAG permitting my child to participate in the Activities; I hereby release, discharge and agree to indemnify and hold harmless the Released Parties from any and all claims; liabilities or cause of action arising out of such treatment and with respect to the exercise of its judgment in this regard. I further attest that I have disclosed all vital and important health information (allergies. medications and medical limitations on activities) which would be necessary for the proper care of my child. I agree to pay, and to assume responsibility for all medical and dental expenses incurred in the treatment of my child. GOVERNING LAW This release shall be construed, interpreted, and my rights will be determined, in accordance with the laws of the state of Arizona without reference to its choice of law provisions and any legal action relating to or arising out of this Waiver and Consent Form shall be commenced exclusively in the Courts of Maricopa County, Arizona. AND I SPECIFICALLY WAIVE THE RIGHT TO TRIAL BY JURY.
